Starting on Tuesday, May 10, 2016, the form for JHH (East Baltimore locations of School of Medicine, Bloomberg School of Public Health, School of Nursing, Johns Hopkins Hospital, Johns Hopkins Community Physicians, Peabody, and Johns Hopkins Bayview Medical Center) Telephone Service Requests will change. The new form is part of a brand new work order system designed to make submitting, receiving and processing telecom service requests simpler and more efficient for customers and staff. Once you have completed the form and successfully submitted, you will receive an e-mail confirmation with the associated work order (WO) number to refer to if you have any questions or have changes to your original submission. The recommended web browsers for accessing the form are Chrome, Firefox, or Internet Explorer 11.
PLEASE NOTE: The process for entering Telecom Trouble Reports (for repair or telephone outage issues) is unchanged. You will still contact the Johns Hopkins Help Desk (410-955-HELP) to report telephone repair issues.
